Immunization Chart, PDF file
What is an “immunization?”
An immunization is a shot that protects your children from disease.
Why is it so important?
- Children need these shots to stay healthy.
- Shots protect a child now and in the future by preventing diseases that sometimes kill or permanently injure children and adults.
- In Oregon, the law requires shots for children. Children must be immunized in order to go to school, daycare or HeadStart programs.
How do I get my child immunized?
- By making an appointment with your child’s Primary Care Provider (PCP).
If your child does not have a PCP, call FamilyCare Customer Service. We can help you find one!
Call us at (503) 222-2880 or (800) 458-9518.
For TTY call (800) 735-2900.
Summer is the best time to get your child shots. Getting an appointment with your child’s PCP can take a few weeks, so don’t wait until September to call for an appointment. Remember, Oregon children cannot attend school if they have not had their shots. |
