A Drug List/Formulary contains drugs and
supplies approved for use with your benefit plan. Our plan
has a closed drug formulary and only medications on the drug
list/formulary are covered on our plan.
If you are currently taking medication that is not listed,
we have a 90 transition policy that allows you time to transition
from your current medication to a formulary medication. We
also have an exception process that allows your provider to
ask us to cover a non formulary medication.
